Government-issued picture ID:
Please bring a valid government-issued picture identification document such as a passport, driver's license, or state identification card. This is required for identification purposes during the medical immigration exam.
Prior immunization records:
If you have any prior immunization records, please bring them along. These records can provide valuable information about your vaccination history and help streamline the evaluation process.
COVID-19 vaccination documentation:
It is essential to bring documentation related to your COVID-19 vaccination, such as your vaccination card.
This will assist in verifying your vaccination status, which is relevant during the immigration medical exam.
Includes: Office visit, blood and urine test (if needed), and consultation fee.
Vaccinations and Chest X-Rays are not included.
1) Tuberculosis Test
This test is used to determine if you have Tuberculosis (TB), and is included within the immigration medical exam price. If you test positive, we will refer you for a chest X-Ray, we do not provide X-Ray services in the office. If you have had a positive TB test in the past and were treated, please bring proof of treatment.
2) MMR Test
This test is only used to indicate whether you have immunity to measles, mumps, and rubella. If you do not have a record of the vaccination or lack immunity you'll need the vaccine. If you require the vaccine the cost is $110.
3) Syphilis (RPR) Test
The RPR test is used to check if you have had syphilis in the past. If you have been treated for syphilis and have proof of treatment, please notify the front desk. This exam is only for patients between the ages of 18-44 years old.
4) Gonorrhea Urine Test
This test is performed to check if a patient has a gonorrhea infection. This exam is only for patients between the ages of 18-24 years old.
Children are required to have their vaccination record up to date including their COVID-19 vaccine.
Adults are required to have the following vaccines: Tetanus, Hepatitis B, COVID-19, and MMR. The Flu vaccine is required if it is Flu season (October - March).

3) Hepatitis B - $80
4) Pneumonia - $250 (Only for people 65 years and older)
5) Flu - $40
* You may visit your primary doctor or local pharmacy for immunizations, just bring proof to your appointment*
